"A decision is a commitment to action under uncertainty.
In an on-call escalation with partial signal, the conversation keeps returning to what you do not know. The practical question is what you will do anyway.
When the decision stays implicit, the system still commits—by drift, escalation, or whoever speaks last. You see it in incident updates where leadership asks for certainty you cannot honestly give, where the first clear call happens after the cost has already been paid. Be honest about what would change your mind, because without a revisit signal every decision becomes permanent by default. If the exception path is informal, it becomes politics; write the signal that makes an exception legitimate and who can approve it.
Decide the default action and the condition that changes it. That is how you turn stress into clarity instead of theater. Write it so execution is obvious.
